Overview

Stool culture uses selective/differential media to recover Salmonella, Shigella, Campylobacter, and other pathogens included in the local enteric panel.

Specimen Requirements

  • Specimen: fresh stool (or rectal swab only when SOP explicitly accepts it)
  • Container: clean leak-proof container; Cary-Blair or other transport medium when required for culture
  • Handling: deliver promptly; follow separate rules for C. difficile testing vs bacterial culture
  • Reject: formed stool may be inappropriate for some diarrheal pathogen pathways; follow SOP

Analytical Method

Stool is inoculated to selective agars (e.g., MacConkey, XLD/HE, Campylobacter media) ± enrichment broths. Suspicious colonies are identified biochemically/MALDI-TOF and confirmed as required. Many laboratories now use multiplex NAAT panels alongside or instead of culture for some pathogens.

Reference Values

Microbiology results are interpreted against organism identification, specimen quality, clinical context, and current CLSI and/or EUCAST interpretive criteria (including CLSI M100 for susceptibility where adopted). Reporting conventions and critical-notification lists are institution-defined; verify against the performing laboratory SOP and current guideline editions.

Expected: no enteric pathogens detected on the ordered panel. Report format lists pathogens included and excluded.

Normal stool flora is not fully catalogued on routine reports.
