Overview
A KOH preparation digests host cellular material so hyphae, yeast, or other fungal structures become more visible on wet microscopy.
Clinical Importance
Rapid screening for dermatophytes and other fungi in skin, hair, nail, or mucosal specimens
Support empiric antifungal decisions while culture is pending
Specimen Requirements
- Specimen: skin scrapings, nail clippings, hair, or other material as ordered
- Container: clean container or slide; avoid heavily contaminated collection
- Handling: examine promptly; some laboratories add DMSO or use calcofluor white with fluorescence
Analytical Method
Specimen is mixed with 10–20% KOH (± DMSO), coverslipped, and examined for refractile fungal elements. Calcofluor white may be added for fluorescence enhancement of chitin-containing structures.

Reference Values
Microbiology results are interpreted against organism identification, specimen quality, clinical context, and current CLSI and/or EUCAST interpretive criteria (including CLSI M100 for susceptibility where adopted). Reporting conventions and critical-notification lists are institution-defined; verify against the performing laboratory SOP and current guideline editions.
No numeric reference interval. Report presence/absence and morphology of fungal elements (hyphae, yeast, pseudo-hyphae).
Interpretation
Common Causes of Increased Results
- Fungal elements seen; correlate with culture/clinical site; artifact and fiber must be excluded
Common Causes of Decreased Results
- Negative KOH does not exclude fungal infection; culture remains more sensitive for many specimens

- Suspected dermatophytosis and superficial mycoses
- Selected mucosal candidiasis evaluations as an adjunct

- Insufficient scraping depth or wrong anatomic site
- Recent topical antifungals reducing visible organisms

- Overcalling artifact as fungus without confirmation
