Malaria blood films use Giemsa-stained thick films for sensitive detection and thin films for species identification and parasitemia estimation.

Specimen Requirements
- Specimen: whole blood
- Tube: lavender-top K₂EDTA (preferred) or K₃EDTA
- Anticoagulant: EDTA
- Handling: invert gently 8–10 times immediately after draw; do not shake
- Storage: room temperature preferred for prompt analysis; follow analyzer/manufacturer limits
- Stability: analyze as soon as practical (many CBC parameters are stable for several hours at room temperature; verify local procedure). MPV increases with EDTA storage time. Do not freeze whole blood for CBC.
- Prepare films promptly . EDTA storage rapidly distorts parasite morphology
- Capillary or venous blood acceptable; collect during febrile episodes when possible
Analytical Method
Thick films concentrate parasites for screening; thin films preserve RBC morphology for speciation (P. falciparum, P. vivax, P. ovale, P. malariae, P. knowlesi). Multiple fields are examined under oil immersion. Rapid antigen tests and NAAT may complement but do not fully replace microscopy for speciation/density in many settings.

Reference Values
Typical adult SI examples below follow widely published international hematology practice (e.g., Dacie and Lewis; Henry's). Always interpret against the method- and demographics-specific interval printed on the patient report (CLSI EP28-A3c). Neonatal, pediatric, pregnancy, and altitude intervals differ.
Expected: no parasites seen. Positive films report species and parasitemia (% infected RBCs or parasites/µL) per laboratory convention. CLSI EP28-style numeric intervals do not apply; interpret against microscopic findings and clinical context.
Interpretation
Critical and significant-risk limits are institution-defined (CLSI GP47). Confirm the local call list. Textbook examples are not universal thresholds.

- Parasites seen; identify species and quantify density; urgent clinical notification for falciparum/high-density disease per policy

- No parasites seen; does not exclude malaria (low density, prior therapy, poor film quality); repeat smears and/or antigen/NAAT as indicated

- Fever in returning travelers / endemic exposure
- Monitoring clearance on therapy in selected protocols
